If you do not receive them:

For Adults age 19 and over, download the Adult Intake Form, and all Consent Forms.

For all patients 18 and under, documentation must be signed by the parent or guardian.

For Children, age 12 and under, download the Child Intake Form, and all Consent Forms.

We require 24 hours’ notice for all appointment cancellations.

If for some reason you have not received the required forms before your appointment, please download and fill out these forms, sign them, and bring the completed forms with you to your initial appointment.
